Antidiuretic Hormone (ADH) and ECF Regulation

ADH Secretion and Action:

• ADH controls water excretion and ECF volume.

• Synthesized in hypothalamus; released by posterior pituitary.

• Acts on collecting ducts of kidneys.

• Increases water absorption in kidney.

• Decreased synthesis during weightlessness and after consumption of alcohol.

ADH-Induced Disease:

• Diabetes insipidus: no effect of ADH; elevated extracellular osmolarity, but dilute, glucose-free urine production.

• Central neurogenic diabetes insipidus: no sufficient ADH.

• Peripheral diabetes insipidus: kidneys insensitive to ADH.

Syndrome of Inappropriate Antidiuretic Hormone secretion (SIADH):

• Excessive ADH secretion causes excessive water reabsorption in renal collecting duct.

Atrial Natriuretic Peptide:

• Released from right atrium of heart.

• Increased due to: stretch, and increased Na.

• Causes: H2O and Na loss by kidney.

• Increased secretion in weightlessness.
