Genus: Legionella.

• Weakly G- pleomorphic rod requiring cysteine and iron.
• Loves water.

L. pneumophila:

• Grows on charcoal-yeast extract (CYE).
• Diagnosis: direct fluorescent antibody; + Dieterle silver stain.

• Found in rivers, streams, cooling tanks.

• Transmitted only by air-conditioning; no human-human.

• Smokers/alcoholics over 55 affected most; immunosuppressed patients.

Pathogenesis:
• Facultative intracellular pathogen; endotoxin.

Diseases:
• Legionnaires' Disease: pneumonia, mental confusion, diarrhea.
• Pontiac fever.

Treatment: fluoroquinolone or azithromycin or erythromycin with rifampin.
